Care Management

Health Home services for children

Baker Victory Services care management services follow a Home Health model to coordinate all services for children enrolled in our program, including behavioral health, medical and social services, and other care.  Our professional staff works to see that all of a child’s caregivers communicate with one another so that the patient’s needs are addressed in a comprehensive manner.

Home Health services are provided through a network of organizations, health plans and community-based providers. Our experienced care managers oversee and provide access to all of the services a child needs to remain healthy and stay out of the emergency room or hospital. Our coordination makes it easier for people with chronic conditions to receive the right combination of preventive services to improve overall health and reduce healthcare costs.
